Prior to your initial consultation, we will send you a detailed information packet which includes forms to be completed before you visit the office. These forms include: patient and diet history along with insurance information. At your initial consultation, Dr. Valentine will determine whether you are a candidate for weight loss surgery and you and Dr. Valentine will discuss which weight loss surgery would be best for you. You will meet Dr. Valentine and his physician assistant Annie who will discuss your medical and diet history as well as different weight loss surgery procedures. You will also discuss the risks and complications associated with weight loss surgeries.
Insurance or Payment Determination
Once it has been determined that you are a candidate for weight loss surgery and you have decided to move ahead with the procedure, we will begin the preauthorization process with your insurance carrier.
Weight loss surgery is covered by many insurance policies, yet coverage can vary from policy to policy. While surgery may be covered by your policy, the authorization or approval process can be intensive and time consuming. We know you may be eager to schedule you surgery right away, but we must complete the authorization process, which can take up to three months before surgery can be scheduled. Our staff will be happy to assist with this process, but please note that it will be your responsibility to obtain any information necessary to meet your insurance’s criteria.
To achieve the best long term results, thorough preparation is needed before the surgery. To ensure that you are fully prepared for all aspects of this procedure, Dr. Valentine requires a psychological evaluation and dietary assessment along with attendance at our educational seminar and at least one support group meeting. Additional tests and consultations may be required depending on individual circumstances.
